Job summary

South Hadley Rehabilitation and Nursing Center in South Hadley, MA is seeking a Full-time Physical Therapist to join our new in-house therapy team. The PT will perform initial evaluations, develop personalized plans, and deliver services to restore mobility and independence.

Collaboration with nurses, PTAs, and physicians is essential, with emphasis on documentation and quality care. The ideal candidate has a DPT/MPT/BSPT, current MA PT license, CPR, and strong clinical and interpersonal skills.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Current State Physical Therapy license in good standing (or eligible).
• Current CPR certification or willingness to obtain upon hire.

Responsibilities

• Perform initial physical therapy evaluations and create personalized treatment plans based on patient goals and needs.
• Deliver physical therapy services to restore mobility, reduce pain, and improve physical function and independence.
• Collaborate with an interdisciplinary care team including nurses, occupational therapists, speech therapists, and physicians.
• Educate patients and families on home exercise programs, safety strategies, and wellness practices.
• Supervise Physical Therapist Assistants (PTAs) and ensure adherence to treatment plans.
• Maintain accurate and timely clinical documentation in accordance with state and federal regulations.
• Participate in care planning, team meetings, and quality improvement initiatives as needed.
