Job summary

Chicopee Rehabilitation and Nursing Center seeks a PRN Physical Therapist to join our in-house team. You will assess referred residents, design and implement treatment plans, and collaborate with physicians and care staff to promote functional recovery.

Open to new grads with PRN shifts, daily Pay option, and a flexible schedule. We value a compassionate approach and compliance with HIPAA and resident rights.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold a valid professional license and registration for the state of practice.
• Experience in long-term care or geriatric care is preferred.
• Experience with Case Mix Index and PPS scheduling is preferred.

Responsibilities

• Assess referred patients needing rehabilitation.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and restrictions.
• Establish and administer a treatment program with goals per patient capacity and physician direction.
• Set realistic and achievable goals for patients.
• Document and educate patients and families in PT methods.
• Complete evaluations, weekly progress notes, recertifications and discharge documents.
• Collaborate with all disciplines for patient-centered care.
• Perform other duties assigned by the Director of Rehabilitation.
• Maintain confidentiality, HIPAA and residents' rights compliance.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services.

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.

• Set realistic and achievable goals for their patients

• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.

• Evaluations, weekly progress notes, recertification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Requirements/Qualifications:
