Job summary

Remedy Home Health Care in Ventura County seeks a licensed Physical Therapist (PT) to provide skilled rehab in home health, assisted living, and community settings. You’ll evaluate patients, develop care plans, and help seniors regain mobility in their homes and local communities.

Qualifications

Licensed Physical Therapist in California (required).
Current CPR certification (AHA approved).
Minimum 1 year in home health or geriatrics preferred.
Valid driver's license and reliable vehicle.

Responsibilities

Conduct in-home PT evaluations and reassessments.
Develop and implement personalized rehabilitation plans.
Improve strength, balance, mobility, and functional outcomes.
Provide therapy in home health, assisted living, and community settings.
Educate patients and families on fall prevention and safe mobility techniques.
Document visits using EMR (WellSky/Kinnser).
Collaborate with interdisciplinary home health teams.

Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients.
