Job summary

Ascension is seeking a licensed Physical Therapist at Ascension St John Sapulpa in Sapulpa, OK. The role focuses on evaluating and treating patients, creating and updating care plans, and supervising PT assistants and students.

The position requires Oklahoma PT licensure, a Bachelor's degree, and prefers a Master’s or Doctorate. This on-site role is part of Rehabilitation Services, with PRN/Days schedule options and a mission-focused workplace culture.

Qualifications

• BLS certification obtained within 30 days of hire.
• PT credentialed from the Oklahoma Board of Medical Licensure and Supervision before or at hire date.
• Bachelor's degree is required; Master's or Doctorate preferred.

Responsibilities

• Evaluate and conduct medically prescribed physical therapy treatment programs.
• Perform initial and ongoing assessments of patient’s condition and adjust plans accordingly.
• Establish, revise and evaluate a plan of care with patient/family involvement.
• Oversee activities of physical therapy assistants, students and support personnel.

How you’ll make an impact in this role

• Evaluate and conduct medically prescribed physical therapy treatment programs. Perform initial and on-going assessments of patient's condition.
• Establish, revise and evaluate a plan of care which is appropriate to problems identified and involves the patient/family.
• Perform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
• Oversee activities of physical therapy assistants, students and other support personnel.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Physical Therapy (PT) credentialed from the Oklahoma Board of Medical Licensure and Supervision prior to hire date or job transfer date required. Physical Therapist temporary letter will be accepted until license is issued.

Education

• Bachelor's degree required. Master's OR Doctorate degree preferred.
