Job summary

Best Home Health Providers, Inc. is seeking a per diem Physical Therapist to deliver skilled home health care across the Bay Area. You will evaluate functional status, mobility, gait, and ROM to craft individualized plans and support patients in regaining independence in their homes.

Responsibilities include collaborating with physicians, educating patients and families, supervising aides when needed, and meticulous documentation. A CA PT license, CPR, and a valid driver’s license are required.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop treatment plans.
• Deliver PT services per physician’s plan of care and California PT Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $160 - $180 per visit.

Responsibilities

• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
