Job summary

Best Home Health Providers Inc. is seeking a per diem Physical Therapist to deliver skilled care in patients' homes across California.

You will assess mobility, design treatment plans, and guide families while coordinating with the medical team. Candidates must hold a Bachelor’s or Master’s in Physical Therapy, have an active California license, at least 2 years of PT experience (home health a plus), CPR certification, and a valid driver’s license with reliable vehicle.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills with patients and families.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and oversee Home Health Aide care bi-weekly when PT is the sole skilled service.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.

Please note that this role is per diem with a rate of $160 - $180 per visit.
