Job summary

Santa Clarita Post Acute Care Center in Santa Clarita, CA seeks a compassionate Licensed Physical Therapist to join our rehabilitation team. You will evaluate residents, develop individualized plans, and provide skilled PT interventions to improve strength, mobility, balance, and independence.

You will collaborate with physicians, nursing, OT, speech therapy, and other professionals, document accurately, and assist with discharge planning to ensure safe transitions of care.

Qualifications

• Must be licensed as a Physical Therapist in California.
• Graduate from an accredited Physical Therapy program.
• New graduates are welcome to apply.
• Post-acute care or skilled nursing experience is preferred but not required.

Responsibilities

• Perform initial evaluations and ongoing assessments to identify resident needs, functional limitations, and goals.
• Develop and implement individualized plans of care.
• Provide PT interventions, including therapeutic exercise, balance training, transfer training, gait training, and mobility training.
• Monitor resident progress and modify treatment plans as clinically appropriate.
• Collaborate with physicians, nursing, OT, speech therapy, and other team members.
• Communicate effectively with residents, families, caregivers, and healthcare professionals.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in discharge planning and provide recommendations for safe transitions of care.

At Newhall Avenue, our Santa Clarita Post Acute Care Center is dedicated to providing a continuum of care that bridges the gap between hospital and home. We are seeking a compassionate and skilled Licensed Physical Therapist (PT) to join our experienced rehabilitation team.

About the Role

As a Physical Therapist, you will work closely with our interdisciplinary team to provide high-quality, patient-centered rehabilitation services. You will evaluate residents, develop individualized plans of care, provide skilled physical therapy interventions, and help residents improve their strength, mobility, balance, safety, and overall functional independence.

Requirements

• Must be a currently licensed Physical Therapist (PT) in the State of California


• Graduate of an accredited Physical Therapy program

• Strong clinical judgment, communication, and interpersonal skills
