Job summary

Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our Home Health team in Orange County, CA. This Per Diem / PRN role offers flexibility and autonomy to provide high-quality rehab and community-based care directly to patients in their homes.

You will work with senior and geriatric patients, delivering skilled therapy in home health, assisted living, and community settings to help patients regain independence and improve quality of life.

Qualifications

• Licensed Physical Therapist (PT) in California (Required).
• CPR certification (AHA approved).
• Minimum 1 year of clinical experience (Home Health preferred).
• Experience in senior care, geriatrics, rehab, or community-based care preferred.
• Strong clinical, communication, and patient education skills.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments.
• Develop and implement personalized rehabilitation care plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy in home health, assisted living, and community care settings.
• Educate patients and families on fall prevention and safe mobility techniques.
• Document visits using WellSky (Kinnser) EMR.
• Collaborate with interdisciplinary home health teams.

Your Role: Home Health Physical Therapist (PT)
