Job summary

Remedy Home Health Care is seeking a licensed Physical Therapist (PT) to join our Home Health team serving Los Angeles County. This Per Diem/PRN role offers flexibility, autonomy, and the chance to provide high-quality rehab and community-based care in patients' homes.

The PT will work with senior and geriatric patients, delivering skilled therapy in home health, assisted living, and community settings. California licensure and CPR certification are required, with 1 year of clinical experience

Qualifications

• Licensed PT in California required.
• CPR Certification (AHA approved) required.
• Minimum 1 year of clinical experience; home health preferred.
• Experience with senior care, geriatrics, rehab or community-based care preferred.
• Strong clinical, communication, and patient education skills.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Conduct in-home physical therapy evaluations and reassessments.
• Develop and implement personalized rehabilitation care plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy in home health, assisted living, and community care settings.
• Educate patients and families on fall prevention and safe mobility techniques.

Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients.
