Job summary

Berkshire Rehabilitation & Skilled Care Center in Sandisfield, MA is seeking a Physical Therapist to join our part-time and per diem team. You will conduct patient assessments, develop care plans, provide direct treatment, and support caregivers within a 57-bed all-male skilled nursing facility.

Physical Therapist Part-time and Per Diem - Berkshire Rehabilitation & Skilled Care Center

We are seeking a dynamic and caring Physical Therapist to join our per diem team at Berkshire Rehabilitation & Skilled Care Center, an award-winning skilled nursing facility in Sandisfield.

We are a 57-bed all-male nursing home with a large veteran population and a majority of long-term stay residents. Up to $55 an hour.

Job Summary:

As a Physical Therapist, you will provide patient/resident assessment, care planning, direct treatment, and consultation to a general rehab caseload. You will also participate in the clinical team process, providing teaching and training to caregiver staff and supervising other therapists, assistants, aides, and students as required.

Responsibilities:

• Screen patients/residents


• Evaluate patients/residents and develop appropriate care plans


• Provide direct resident treatment in accordance with an established plan of care


• Consult with and train patients, residents, and caregivers


• Monitor efficiencies on an ongoing basis in collaboration with supervisor


• Document activities according to the company's clinical documentation system on a timely basis


• Demonstrate consistent knowledge and application of Medicare and other reimbursement regulations


• Communicate effectively with patients, residents, team members, customers, and supervisor


• Participate in clinical program development and implementation


• Participate in the training and orientation of other staff, students, and volunteers


• Participate in the interview process for new staff


• Participate in census building and marketing activities


• Participate in quality improvement activities

Requirements:

• Bachelor of Science Degree, Master of Science or Doctoral degree in Physical Therapy


• Appropriate licensing requirements by individual states must be maintained


• Ability to function effectively in multi-faceted systems and to interact effective with all levels of staff and customers
