Job summary

Carmel Mountain Rehab & Care in San Diego, CA is seeking a full-time Physical Therapist for our in-house rehab program. You will assess patients, develop treatment plans, and deliver rehabilitative interventions to support recovery in a geriatric post-acute setting.

Licensed Physical Therapist required; new grads welcome. This on-site role offers a Tuesday–Saturday schedule with pay between $50 and $55 per hour, commensurate with experience, plus a comprehensive benefits package.

Qualifications

• Physical Therapist license is required.
• Open to Physical Therapists at all experience levels.

Responsibilities

• Conduct screening and evaluation of patients with physical function disorders or wound care, developing care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer procedures as part of the rehabilitation plan (manual techniques, ambulation, therapeutic exercises, modalities, assistive devices, wound care).
• Provide consultation to patients, families, and other providers regarding physical disorders.
• Generate comprehensive discharge summaries ensuring regulatory compliance.

Carmel Mountain Rehab & Care in San Diego, CA, is a post-acute care facility with long‑term care, short‑term care, and geriatric services provided. We can offer a stable and fun in‑house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in‑house rehab program!

Full Time: Tuesday - Saturday availability

Pay : $50 - $55 /hour based on experience

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
