Job summary

Del Rosa Villa Post Acute in California is seeking a licensed Physical Therapist (PT) for per diem/part-time hours. Compensation is $50.00-$60.00 per hour DOE.

Work in a supportive post-acute setting with a collaborative rehab team. You will evaluate residents, develop individualized treatment plans, provide skilled therapy per physician orders, collaborate with physicians, nursing staff and families, document progress, and educate on mobility and safety to help residents regain independence and

Qualifications

• Must hold a California Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and documentation skills.
• Post-acute or skilled nursing experience preferred but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services in accordance with physician orders and regulatory guidelines
• Collaborate with physicians, nursing staff, therapy team members, residents, and families
• Monitor patient progress and modify treatment plans as needed
• Complete timely and accurate documentation
• Educate residents and caregivers on mobility, safety, and recovery strategies
• Support residents in achieving maximum independence and quality of life

What We're Looking For
