Job summary

Corewell Health is seeking a skilled Physical Therapist for Lakeland Hospital St Joseph, delivering therapy based on patient needs under medical direction. Responsible for patient education and arranging adaptive equipment as needed.

The role includes evaluating patients, developing treatment plans, documenting progress, and collaborating with nursing and medical staff to ensure high-quality rehab services.

Qualifications

• Bachelor's Degree or equivalent from an accredited physical therapy program.
• Physical Therapist - State of Michigan.
• BLS Certification from American Heart Association or American Red Cross (within 90 days).
• Preferred Master's Degree with 6 months of relevant experience.

Responsibilities

• Performs evaluation procedures to assess disability level and create a plan of treatment.
• Delivers a treatment program to restore maximal function.
• Assesses treatment effectiveness and adjusts goals accordingly.
• Documents evaluation and treatment per department standards and reimbursement procedures.
• Instructs and trains rehabilitation staff and students on patient needs and care.
• Participates in process improvement, patient satisfaction, and program development activities.
• Demonstrates productivity per department standards.
• Advances skills through continuing education and competencies.

Education

Bachelor's Degree or equivalent from an accredited physical therapy program

Master's Degree in Physical Therapy

BLS Certification (American Heart Association or American Red Cross)

Job description

Job Summary

Provides therapy to patients based on patient need and medical direction. Provides patient/caregiver education and arranges for adaptive equipment as needed.

Essential Functions

• Performs evaluation procedures to assess the level of disability with regard to scope of practice, goals and a plan of treatment, and to make appropriate recommendations.
• Provides a treatment program and education to restore and return maximal function to the patient.
• Assesses effectiveness of treatment and modifies goals / treatment methods, as needed to meet changing needs.
• Documents evaluation and treatment of each patient in compliance with department standards, medical-legal and reimbursement procedures.
• Instructs and trains other rehabilitation staff, nursing, other medical personnel and students regarding patient needs and rehabilitation services.
• Participates in ongoing process improvement, patient satisfaction initiatives/improvement, program development and marketing activities.
• Demonstrates productivity as indicated by department standard.
• Improves skill areas as demonstrated by applying continuing education in-services, participating in competencies in applicable areas, and developing new concepts of patient care and additional skills with various population groups.

Qualifications

• Bachelor's Degree or equivalent from an accredited physical therapy program
• Physical Therapist - State of Michigan
• Upon Hire Basic Life Support (BLS) - American Heart Association 90 Days Or Basic Life Support (BLS) - American Red Cross 90 Days
• Preferred Master's Degree 6 months of relevant experience

Contract and Work Schedule

Corewell Health is offering up to $15,000 in sign on bonuses for this opportunity! Expected to work 4 9 hours days Monday to Friday plus 1 weekend a month working 8 hour days on the weekend scheduled.

Full time Shift Day (United States of America) Weekly Scheduled Hours 40 Hours of Work 8 a.m. to 5:30 p.m. Days Worked Variable Weekend Frequency Every fourth weekend
