Job summary

Midtown Oaks Post Acute in Sacramento, CA is seeking a Physical Therapist to join our rehabilitation team. We welcome both new graduates and experienced clinicians to provide skilled PT services in a skilled nursing/post-acute setting.

You will evaluate residents, develop individualized plans, and deliver evidence-based therapy while documenting progress and collaborating with physicians and staff. We offer a $7,500 sign-on bonus, flexible scheduling, mentorship, and growth opportunities.

Qualifications

• Active California Physical Therapist license or eligibility.
• Experience in skilled nursing, rehabilitation, or post-acute care preferred.
• Strong documentation and time-management skills.

Responsibilities

• Evaluate residents and establish functional goals.
• Develop and implement individualized treatment plans.
• Deliver evidence-based physical therapy services in SNF/post-acute care.
• Complete evaluations, progress notes, discharge summaries, and documentation.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members.
• Educate patients and caregivers on treatment plans and home exercise programs.
