Qualifications

Responsibilities

  • Monitor and adjust patient visit schedules to optimize productivity and meet patient needs.
  • Perform comprehensive assessments including psychosocial, educational, safety, and risk factors.
  • Develop and document individualized plans of care with patient-specific goals and interventions.
  • Provide skilled physical therapy treatments and patient/caregiver education.
  • Coordinate patient care with physicians, nurses, caregivers, and other rehabilitation team members.
  • Supervise and direct activities of PT assistants, students, and home care aides.
  • Complete and maintain required clinical documentation in compliance with federal, state, and organizational standards.
  • Ensure accurate billing and insurance documentation and address coverage inconsistencies.
  • Participate in training programs for staff, volunteers, and community groups.
  • Maintain and enhance professional knowledge and standards in physical therapy.
  • Engage in marketing and promotion of rehabilitation services.
  • Perform various patient health assessments and procedures such as PT/INR, blood pressure, wound care, and staple/suture removal.
  • Communicate schedule changes promptly to supervisors and schedulers.
  • Complete OASIS documentation and collaborate with Quality Improvement staff to ensure accuracy.
  • Follow agency protocols for documentation and visit management using McKesson systems.
  • Assess and coordinate Home Care Aide services as needed.
  • Coordinate safe patient discharges and transfers with proper documentation.
  • Report patient incidents timely to Quality Improvement specialists.
  • Participate in peer review audits and provide constructive feedback.

Job Summary:
