Job summary

Prestige Care Center Of Plattsmouth in Plattsmouth, NE is seeking a compassionate Physical Therapist (PT) for per diem/PRN to join our rehabilitation team. You will evaluate residents and develop individualized plans to restore function and independence, delivering skilled PT services in a supportive skilled nursing environment.

This role offers flexible scheduling (25–30 hours per week) with consistent hours and a collaborative interdisciplinary team focused on mobility and quality of life.

Qualifications

  • Current Nebraska Physical Therapist license (unrestricted).
  • Graduate of an accredited Physical Therapy program.
  • CPR certification (preferred).
  • Valid driver's license.
  • Knowledge of skilled nursing regulations, Medicare documentation, and long-term care best practices.
  • Strong communication, clinical reasoning, and documentation skills.

Responsibilities

  • Evaluate residents and develop individualized treatment plans to restore function and independence.
  • Provide skilled PT services per physician orders, regulations, and APTA standards.
  • Assess gait, balance, strength, endurance, ROM, transfers, and mobility with measurable goals.
  • Implement evidence-based interventions to improve outcomes and reduce fall risk.
  • Complete all clinical documentation, billing, and coding accurately.
  • Collaborate with physicians, nursing staff, and other therapists to coordinate care.
  • Participate in care conferences, discharge planning, and interdisciplinary meetings.
  • Educate residents and families on exercises, mobility, fall prevention, and home programs.
  • Monitor progress and adjust plans to achieve functional goals.
  • Maintain therapy equipment and treatment areas in a safe, organized manner.

Required Qualifications

Key Responsibilities
  • Evaluate residents and develop individualized treatment plans designed to restore function, improve mobility, reduce pain, and maximize independence.

  • Provide skilled physical therapy services in accordance with physician orders, state and federal regulations, APTA standards, and facility policies.

  • Assess gait, balance, strength, endurance, range of motion, transfers, and functional mobility while establishing measurable treatment goals.

  • Implement evidence-based interventions to improve functional outcomes and reduce fall risk.

  • Accurately complete all clinical documentation, billing, and coding in compliance with facility and payer requirements.

  • Collaborate with physicians, nursing staff, occupational and speech therapists, social services, and other interdisciplinary team members to coordinate resident care.

  • Participate in care conferences, discharge planning, and interdisciplinary meetings.

  • Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, assistive device use, and home exercise programs.

  • Monitor resident progress, modify treatment plans as appropriate, and ensure achievement of functional goals.

  • Maintain therapy equipment and treatment areas in a clean, safe, and organized manner.

  • Adhere to the APTA Code of Ethics, state practice regulations, and facility policies while maintaining the highest standards of professional conduct.
