Good Shepherd Lutheran Community is seeking a compassionate, skilled, and motivated Physical Therapist (PT) to join our rehabilitation team in Full-Time or Per Diem capacity. This is an excellent opportunity for a dedicated therapist who is passionate about helping residents improve their mobility, strength, balance, functional independence, and overall quality of life.
Our Physical Therapists work closely with residents, families, nursing staff, physicians, and other rehabilitation professionals to provide individualized, resident-centered care in a supportive community environment.

Responsibilities
- Evaluate residents' physical function, mobility, strength, balance, endurance, and functional limitations.
- Develop individualized physical therapy treatment plans based on each resident's clinical needs, goals, and level of function.
- Provide skilled therapeutic interventions to improve strength, balance, coordination, endurance, mobility and functional independence.
- Perform gait training, transfer training, therapeutic exercises, neuromuscular re‑education, and functional mobility training.
- Assess and recommend appropriate assistive devices, adaptive equipment, and mobility strategies.
- Provide education to residents, families, and caregivers regarding exercises, mobility techniques, fall prevention, safety, and home recommendations.
- Monitor resident progress and modify treatment plans as clinically appropriate.
- Collaborate with physicians, nursing staff, occupational therapists, speech‑language pathologists, and other members of the interdisciplinary team.
- Participate in care planning, interdisciplinary meetings, discharge planning, and other clinical activities as appropriate.
- Maintain accurate, timely, and thorough clinical documentation in accordance with facility policies and regulatory requirements.
- Promote resident safety, dignity, independence, and quality of life throughout the rehabilitation process.
- Maintain professional standards and comply with applicable state and federal regulations.
Qualifications
- Doctor of Physical Therapy (DPT) or Bachelor's/Master's degree in Physical Therapy from an accredited program.
- Current Nebraska Physical Therapist license in good standing.
- Strong clinical reasoning, communication, organizational, and time‑management skills.
- Ability to work effectively with older adults and individuals with varying levels of physical and functional ability.
- Experience in skilled nursing, senior living, rehabilitation, geriatrics, or a similar setting is preferred but not required.
- New graduates with appropriate licensure are welcome to apply.

- Ability to work independently while collaborating effectively with an interdisciplinary team.
