Job summary

Prestige Care Center of Plattsmouth is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in Plattsmouth, NE. This per diem position offers flexibility with a consistent schedule of 25–30 hours per week.

You will evaluate residents, develop individualized treatment plans, and provide skilled PT services in a collaborative, resident-centered environment. The role requires Nebraska licensure, an accredited PT program, and strong documentation skills.

Qualifications

  • Current, valid, unrestricted Physical Therapist license in Nebraska.
  • Graduate of an accredited Physical Therapy program.
  • M eets all educational and licensing requirements for state practice.
  • Current CPR certification (preferred).
  • Valid driver's license.
  • Knowledge of skilled nursing regulations, Medicare documentation requirements, and long-term care best practices.
  • Strong communication, clinical reasoning, and documentation skills.

Responsibilities

  • Evaluate residents and develop individualized treatment plans designed to restore function, improve mobility, reduce pain, and maximize independence.
  • Provide skilled physical therapy services in accordance with physician orders, state and federal regulations, APTA standards, and facility policies.
  • Assess gait, balance, strength, endurance, range of motion, transfers, and functional mobility while establishing measurable treatment goals.
  • Implement evidence-based interventions to improve functional outcomes and reduce fall risk.
  • Accurately complete all clinical documentation, billing, and coding in compliance with facility and payer requirements.
  • Collaborate with physicians, nursing staff, occupational and speech therapists, social services, and other interdisciplinary team members to coordinate resident care.
  • Participate in care conferences, discharge planning, and interdisciplinary meetings.
  • Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, assistive device use, and home exercise programs.
  • Monitor resident progress, modify treatment plans as appropriate, and ensure achievement of functional goals.
  • Maintain therapy equipment and treatment areas in a clean, safe, and organized manner.
  • Adhere to the APTA Code of Ethics, state practice regulations, and facility policies while maintaining the highest standards of professional conduct.
