Job summary

Interim HealthCare is seeking a Home Health Physical Therapist (PT) in Michigan to deliver personalized, in-home therapy. You will work with a dedicated home health team to help patients regain mobility, strength and function in their daily lives through customized treatment plans.

The role offers 1:1 therapist-to-patient care, flexibility in assignments, paid vacation, weekly pay, and an ESOP program to build retirement value after eligibility.

Qualifications

• Graduate of an accredited Physical Therapist Program and active PT license in Michigan.
• Minimum 1 year of clinical experience, preferably in home health within last 5 years.
• CPR certification is required.
• Strong clinical judgement and empathy.

Responsibilities

• Provide home-based physical therapy to restore mobility and reduce pain.
• Collaborate with a home health team (RN, LPN, CNA, HHA, SLP) to execute care plans.
• Observe patient movements, assess progress, and adjust plans as needed.
• Educate patients and families on exercises, goals, and self-care.
• Maintain detailed progress notes and ensure safety during visits.
