Job summary

Interim HealthCare of Rochester is seeking a Home Health Physical Therapist to help patients regain strength and manage pain in a home setting. You will work with a compassionate home health team to assess, plan and deliver customized therapy to improve mobility and independence.

Provide patient education, support families, and document progress to ensure the plan of care meets patient goals across Rochester communities. This role offers a generous compensation package and benefits.

Qualifications

• Must have completed an accredited Physical Therapist program and hold an active Minnesota PT license.
• Minimum of 1 year of PT experience, preferably in home healthcare.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Strong interpersonal skills, good clinical judgement, dexterity and compassion.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to illness or injury.
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals.
• Observe movements, conduct assessments, document progress and modify care plan as needed.
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility.
• Educate patients on their plan of care, established goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safely assisting with exercises.
