**Join Our Team as a Compassionate Physical Therapist!**

Delia Maria Home Healthcare Agency is seeking a dedicated and compassionate Physical Therapist to join our growing team. We are committed to providing exceptional home healthcare services and are looking for a skilled professional to support our patients in achieving their highest level of physical well-being.

As a Physical Therapist with us, you will be responsible for reviewing charts, providing direct care, completing OASIS assessments, and handling discharges. You will collaborate with our interdisciplinary team—comprised of our Medical Director (MD), Director of Nursing (DON), Registered Nurses (RN), and Occupational Therapists (OT)—to develop and implement comprehensive care plans tailored to each patient’s unique needs.

What You’ll Do:

• Assess patients and identify areas for improvement to meet goals and optimize functional mobility.
• Deliver cost-effective, evidence-based treatments aimed at improving motion and reducing the need for medications or surgery.
• Offer expert input to the interdisciplinary team to enhance patient care strategies.
• Develop individualized care plans using a variety of treatment techniques.
• Monitor and document progress, ensuring continuous evaluation and communication.
• Educate patients and caregivers on home-based exercises and preventative care.

What We’re Looking For:

* Proven experience as a Physical Therapist, preferably in home health settings.

* Graduate degree or higher in Physical Therapy.

* Valid Physical Therapy license.

* Valid **Pennsylvania Driver’s License** and vehicle registration.

* Strong clinical judgment and ability to work independently.

* Excellent communication and interpersonal skills.

* A patient-centered approach with a passion for rehabilitation and long-term care.

* Minimum commitment of **20 hours per week**.

Pay:$50.00 – $60.00 per hour
