Job summary

VitalCaring is seeking a Home Health Physical Therapist to help patients recover and regain mobility in their homes across Melbourne, FL. You will evaluate functional status, develop care plans, provide hands-on therapy, and educate patients and families, documenting progress in the EMR.

This highly autonomous, relationship-driven role offers meaningful patient impact, collaboration with clinicians, supervision of PTAs, and ongoing clinical education.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians for coordinated care.
• Document patient care and progress accurately in the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) per state guidelines.
• Provide guidance and education to caregivers and support staff.

What You’ll Do

Deliver Patient-Centered Care

• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
