Job summary

VitalCaring Sebring is seeking a Home Health Physical Therapist to join our compassionate team. You will deliver patient-centered care in patients' homes, evaluate mobility, design plans, and help regain independence.

This autonomous, relationship-driven role offers growth, flexible scheduling, and the chance to see real impact daily while collaborating with nurses, therapists, and physicians.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master)
• Current PT license in the state of practice
• Minimum 1–2 years of home health experience (preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver''s license and reliable transportation

Responsibilities

• Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Document patient care and progress accurately in the EMR
• Supervise Physical Therapist Assistants (PTAs) per state guidelines
• Collaborate with nurses, therapists, and physicians for coordinated care
• Participate in case conferences and team meetings

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master's)
• Current PT license in the state of practice
• Minimum 1-2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
