Job summary

VitalCaring, West Palm Beach Branch, is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes. You will evaluate mobility, develop plans with physicians, and provide hands-on therapy to improve independence.

The role emphasizes autonomy, collaboration with a multidisciplinary team, and ongoing clinical education. A valid PT license, CPR, and driver’s license are required, with 1–2 years of home health experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Deliver patient-centered care by evaluating functional status, mobility, strength and rehabilitation potential.
• Develop and implement individualized care plans with physicians and care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises and safety strategies.
• Monitor progress and adjust treatment plans to ensure optimal outcomes.
• Support transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff

What You’ll Do

What You Bring
