Adept Rehab and Nursing of Omaha invites a dedicated Physical Therapist to join our rehabilitation team in a full-time capacity. You will evaluate residents, develop individualized care plans, and provide hands-on therapy per physician orders to improve strength, mobility, and independence.
Responsibilities include documenting progress, coordinating with physicians and nursing staff, and supervising therapy aides. Nebraska PT licensure is required; LTC experience is preferred but not mandatory.

The Banyan at Montclair is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team. This full-time position provides an opportunity to help residents improve their strength, mobility, independence, and overall quality of life.
Evaluate residents using appropriate physical therapy assessment techniques.
Develop and implement individualized plans of care based on each resident’s needs and goals.
Provide hands-on physical therapy services in accordance with physician orders and established treatment plans.
Review resident progress and revise treatment plans as appropriate.
Listen and respond to resident and family concerns when developing or modifying treatment plans.
Educate and motivate residents and their families regarding exercises, mobility, safety, and functional activities.
Complete progress notes, discharge summaries, home programs, and all other required documentation accurately and promptly.
Communicate with physicians, nursing staff, rehabilitation team members, and other healthcare professionals to coordinate resident care.
Provide appropriate direction and supervision to Physical Therapist Assistants and rehabilitation aides.
Maintain resident privacy and confidentiality in accordance with HIPAA requirements.
Ensure compliance with federal, state, facility, and professional standards.
Follow all facility policies, procedures, infection-control practices, and safety requirements.
Participate in continuing education and maintain all required professional credentials.
Build positive and professional working relationships with residents, families, coworkers, and facility leadership.
Demonstrate sound clinical judgment and promptly report concerns or significant changes in a resident’s condition.
Perform other duties as assigned.
Current and unrestricted license as a Physical Therapist in the State of Nebraska.
Graduate of an accredited physical therapy program.
Long-term care or skilled nursing experience is preferred but not required.
Strong clinical assessment, communication, organizational, and documentation skills.
Ability to work effectively as part of an interdisciplinary care team.
Regular and dependable attendance is required.
Ability to walk, stand, sit, bend, reach, and perform other physical activities throughout the workday.
Ability to assist residents safely during transfers, mobility activities, and therapeutic exercises.
Ability to push, pull, and lift up to 50 pounds.
