Job summary

The Banyan at Montclair in Omaha, Nebraska is seeking a compassionate Physical Therapist to join our rehabilitation team. This full-time position aims to help residents improve strength, mobility, independence, and overall quality of life.

Responsibilities include evaluating residents, developing individualized care plans, providing hands-on PT services, documenting progress, and coordinating with physicians and the rehab team. Licensure in Nebraska is required.

Qualifications

• Current and unrestricted Nebraska Physical Therapist license.
• Graduate of an accredited physical therapy program.
• Experience in long-term care or skilled nursing is preferred but not required.
• Strong clinical assessment, communication, organizational, and documentation skills.
• Ability to work effectively as part of an interdisciplinary care team.
• Regular and dependable attendance is required.

Responsibilities

• Evaluate residents using PT assessment techniques.
• Develop and implement individualized plans of care based on each resident’s needs and goals.
• Provide hands‑on physical therapy services in accordance with physician orders and established treatment plans.
• Review resident progress and revise treatment plans as appropriate.
• Educate and motivate residents and their families regarding exercises, mobility, safety, and functional activities.
• Complete progress notes, discharge summaries, home programs, and all other required documentation accurately and promptly.
• Communicate with physicians, nursing staff, rehabilitation team members, and other healthcare professionals to coordinate resident care.
• Provide appropriate direction and supervision to Physical Therapist Assistants and rehabilitation aides.
• Maintain resident privacy and confidentiality in accordance with HIPAA requirements.
• Ensure compliance with federal, state, facility, and professional standards.
• Follow all facility policies, procedures, infection‑control practices, and safety requirements.
• Participate in continuing education and maintain all required professional credentials.
• Build positive and professional working relationships with residents, families, coworkers, and facility leadership.
• Demonstrate sound clinical judgment and promptly report concerns or significant changes in a resident’s condition.
• Perform other duties as assigned.

• Ability to push, pull, and lift up to 50 pounds.
