Innovate Rehab and Wellness in Nortonville, KS is seeking a Licensed Physical Therapist for a full-time role in a skilled nursing facility. You will evaluate patients, identify movement limitations, create and lead individualized treatment plans, and provide hands-on therapy to restore movement and quality of life.

Work with an interdisciplinary team for optimal recovery, stay current with PT best practices, and ensure regulatory compliance.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with regulatory and ethical standards.
