Job summary

Innovate Rehab and Wellness is seeking a licensed Physical Therapist for a full-time position in a skilled nursing facility. The role focuses on one-on-one patient care, evaluating movement limitations, and leading individualized treatment plans to restore function and quality of life.

You will collaborate with an interdisciplinary team, document progress, and stay current with PT best practices through continuing education. Flexible schedule available.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.

Responsibilities

• Perform comprehensive PT evaluations to identify movement limitations.
• Develop and lead individualized, evidence-based treatment plans.
• Provide hands-on therapy including manual techniques and therapeutic exercise.
• Educate patients and caregivers on home programs and prevention.
• Track progress and adjust plans; document outcomes.
• Coordinate with physicians, nurses, and other clinicians.
