Job summary

Horsham Center for Jewish Life is seeking a compassionate Physical Therapist (PT) to join our collaborative therapy team. This position offers Full-Time, Part-Time, or PRN opportunities to help residents improve mobility and quality of life.

You will perform evaluations, develop individualized plans, and work closely with nursing and therapy colleagues. A PT license in good standing is required; APTA-accredited program is preferred; strong communication and teamwork are essential.

Qualifications

• Current Physical Therapist license in good standing is required.
• Graduate of an accredited Physical Therapy program recognized by APTA.

Responsibilities

• Provide skilled physical therapy services to residents.
• Conduct comprehensive PT evaluations based on physician orders.
• Develop individualized treatment plans with short- and long-term goals.
• Provide skilled interventions and modify plans as needed.
• Participate in resident assessments and care plan development.
• Collaborate with nursing and other team members.

What You’ll Do

• Provide skilled physical therapy services to residents throughout the facility


• Conduct comprehensive physical therapy evaluations based on physician orders


• Develop individualized treatment plans with short- and long-term goals


• Provide skilled interventions and modify treatment plans as clinically indicated


• Participate in resident assessments and care plan development


• Collaborate with nursing, therapy, and other interdisciplinary team members

Qualifications

• Graduate of an accredited Physical Therapy program recognized by APTA


• Bachelor’s, Master’s, or Doctorate in Physical Therapy


• Current Physical Therapist license in good standing required


• Strong communication, documentation, and teamwork skills
