This position is Mon-Fri with flexible hours (no nights).

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Responsibilities include:

• Evaluating the patient's functional status and therapy needs
• Assisting the physician in developing the plan of care
• Observing and reporting patient responses and condition changes
• Treating patients to relieve pain, restore, develop, or maintain maximum function
• Instructing patients, families, and healthcare team members about the care plan and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Kentucky
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
