Job summary

Xcelerate Physical Therapy in Delaware County is seeking a dedicated Physical Therapist to join our outpatient clinic. You will assess patients, plan and implement individualized treatment programs, and document progress for ongoing rehabilitation.

New graduates are welcome. A PA license and CPR certification are required, with opportunities for professional development, mentoring, and career advancement in a supportive team environment.

Qualifications

• Graduate from a CAPTE accredited Physical Therapy Program.
• New or recent graduates are encouraged to apply.
• Current PA licensure as Physical Therapist (PT).
• CPR certification.

Responsibilities

• Assess patients to develop and initiate treatment plans.
• Review the physician's referral and patient records to determine therapy needs.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Oversee patient care performed by assistants and rehabilitation technicians.
• Complete a written plan of care and develop goals and a comprehensive program.
• Implement therapy treatment and provide essential physical therapy services.
• Communicate with patients, caregivers, physicians, and insurance companies.
• Ensure timely documentation including notes, progress reports and discharge summaries.

Job Description

Xcelerate Physical Therapy is looking for a motivated individual to join our team as a Physical Therapist.

What Are Some Of The Things You Will Do

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, progress reports, initial evaluations, discharge summaries, and incident reports

Qualifications

• Graduate from a CAPTE accredited Physical Therapy Program
• New or recent graduates are encouraged to apply
• Current state of PA licensure as Physical Therapist (PT)
• CPR certification
• Ability to work efficiently/effectively in an autonomous environment
• Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office
