Job summary

WelbeHealth is seeking a Physical Therapist to join our interdisciplinary team (IDT) delivering restorative and rehabilitative PT services to participants, maximizing independence and safety.

You will conduct assessments, participate in IDT care planning, deliver and document interventions, supervise therapy assistants, and train staff on equipment use.

Compensation includes base pay plus bonus with a comprehensive benefits package; vaccination policy applies.

Qualifications

• Bachelor's degree in Physical Therapy; Master’s or Doctorate preferred.
• Valid New Jersey Physical Therapy License or eligibility.
• Graduation from a CAPTE/APTA accredited program.
• CPR and First Aid certification within 180 days of hire and maintained thereafter.

Responsibilities

• Conduct Physical Therapy assessments in the facility and home visits as needed.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as per care plans.
• Provide active and passive exercises, gait training, ADL training, and various modalities (massage, heat, sound, water, light, electricity).
• Supervise Therapy Assistants and provide training for Functional Maintenance Programs.
• Train staff, participants, and caregivers on use and care of therapeutic equipment.

Job description

At WelbeHealth each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care.

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

On the day-to-day, you will:

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Job requirements include:

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
• Valid New Jersey Physical Therapy License, or proof of license eligibility
• Graduation from a CAPTE/APTA Accredited Physical Therapy Program required
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
