Job Summary

We are looking for a dedicated and skilled Physical Therapist to join our team. The ideal candidate has a strong background in patient care, with experience in various settings and patient populations. We have a one-on-one model of care between our clinicians and patients. This role involves assessing patient needs, developing individualized care plans, and implementing effective interventions to enhance recovery and improve their quality of life.

Responsibilities

• Conduct comprehensive evaluations of patients to determine their physical therapy needs.
• Develop and implement personalized care plans based on individual patient assessments.
• Provide skilled physical therapy interventions including manual therapy techniques, patient education, and exercise interventions.
• Collaborate with fellow healthcare professionals to ensure coordinated patient care.
• Educate patients and their families on treatment plans, exercise programs, and rehabilitation strategies.
• Document patient progress using EMR systems and maintain accurate, HIPAA compliant, medical records.
• Participate in discharge planning to ensure smooth transitions from the clinic to home or further care.
• Stay updated on the latest developments in physical therapy practices and medical technology.

Qualifications

• Doctor of Physical Therapy Degree (DPT) from an accredited program.
• Valid NY State licensure to practice as a Physical Therapist.
• Strong knowledge of anatomy and physiology as it relates to patient care.
• Experience in orthopedics, geriatrics, pediatrics, and outpatient settings is preferred.
• Proficient in using EMR systems for documentation and patient management.
