Job summary

Highland Care Center in Queens, NY is seeking a Full-Time Physical Therapist (PT) to evaluate residents, develop individualized treatment plans, and deliver therapy to improve function and reduce pain.

You will work with an interdisciplinary team in a resident-centered environment, with mentorship for new graduates, a Monday–Friday schedule, and options for 1099 or W2 employment. Benefits available.

Qualifications

• Active NY PT license or eligibility to practice in NY.
• Excellent interpersonal and communication skills.
• Willingness to mentor new graduates and contribute to team.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver physical therapy to relieve pain and improve function.
• Ensure care aligns with established plan of care.
• Educate patients, families, and caregivers on exercises.
• Monitor response to treatment and adjust plans as needed.
• Complete required forms and documentation per policy.
• Collaborate with the interdisciplinary team for resident-centered care.

Responsibilities:

• Evaluate residents and patients with temporary or long-term disabilities and develop individualized treatment plans

• Administer physical therapy treatments to relieve pain, restore or improve function, and promote healing

• Assure all treatment is delivered in accordance with an established plan of care

• Provide clinical support and educate patients, families, and caregivers

• Monitor patient response to treatment interventions and adjust treatment plans as appropriate

• Complete required forms and documentation in accordance with company policy and state/federal regulations

• Collaborate with the interdisciplinary team to provide quality, resident-centered care
