Advanced Center for Nursing and Rehabilitation in New Haven, CT is seeking a licensed Physical Therapist to provide comprehensive rehabilitation services to residents.

You will work with an interdisciplinary team to develop personalized plans, evaluate residents, deliver evidence-based therapy, document progress, and educate families about goals and home exercises, promoting independence and mobility.

Qualifications

• Current and valid CT PT license.
• Graduate of an accredited Physical Therapy program.
• New grads welcome; SNF or geriatric experience preferred.

Position Summary

We are seeking a licensed PT to provide comprehensive rehabilitation services to residents with a variety of medical conditions. The PT will work closely with an interdisciplinary team to develop personalized care plans that help residents regain strength, mobility, and independence.

Key Responsibilities

• Evaluate residents and develop individualized treatment plans
• Deliver evidence-based physical therapy services
• Document progress and adjust treatment plans as needed
• Collaborate with physicians, nursing staff, and other rehab team members
• Educate residents and families on therapy goals and home exercises

Qualifications

• Current and valid Physical Therapist (PT) license in CT
• Graduate of an accredited Physical Therapy program
• Previous experience in SNF or geriatric setting preferred (new grads welcome!)
• Strong communication and interpersonal skills
