Job summary

MercyOne in New Hampton, IA is seeking a licensed Physical Therapist to develop individualized treatment plans for home health clients. You will work with a multidisciplinary team in the patient's home to deliver compassionate, patient-centered care.

Responsibilities include evaluating clients, performing initial assessments, creating plans of care, coordinating with physicians, and setting patient-centered goals with the interprofessional team.

Qualifications

• Graduate of a university approved by American Physical Therapy Association
• Licensed in the state of Iowa to practice physical therapy
• Minimum of 1-year experience preferred
• Community or home health experience desirable

Employment Type: Full time Shift: Day Shift Description: Full-Time - 40 hours weekly; Days - 8 hour shift; New Hampton Medical Center

This individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for client’s home/residence.
