What You Will Do

• Perform in-home physical therapy evaluations and treatments tailored to each patient.
• Develop and implement individualized care plans in collaboration with physicians and care teams.
• Document patient progress and adjust care plans to meet evolving needs.
• Educate patients and their families to support continued progress and improved outcomes.
• Maintain high standards of care and ensure adherence to all safety and clinical protocols.

What You Will Need

• Valid New Jersey Physical Therapy License.
• 2-3 years of clinical experience; community-based or homecare experience preferred.
• Strong communication, assessment, and computer documentation skills.
• Valid NJ driver’s license and reliable transportation.

Working Conditions/Physical Demand

This is a field-based role requiring travel throughout Monmouth County to patient homes. Includes use of clinical tools, technology, and regular interaction with patients, families, and interdisciplinary teams.
