Job summary

LiveWell Homecare Agency, LLC is seeking licensed Physical Therapists to provide in-home and senior living care in Muncie, IN. You will deliver one-on-one therapy to older adults in private residences, assisted living and memory care, focusing on mobility, independence, and quality of life.

You will conduct evaluations, create personalized care plans, and document outcomes in EMR. Collaborate with an interdisciplinary team and educate patients, caregivers and staff to promote aging in place.

Qualifications

• Valid PT license in Indiana.
• Degree from an accredited Physical Therapy program.
• Proficiency with EMR systems and CPR certification.

Responsibilities

• Perform evaluations and develop personalized care plans.
• Deliver evidence-based, one-on-one therapy to older adults.
• Document treatment in EMR and collaborate with the interdisciplinary team.
