Job summary

Pine Crest Health & Memory Care in Merrill, WI is seeking a full-time Physical Therapist to join our in-house rehab program. You will contribute to high-quality patient care and collaborate with an in-house therapy team to support geriatric recovery goals.

Responsibilities include evaluating patients, designing care plans, delivering rehabilitative treatments, and preparing discharge summaries in accordance with regulatory standards.

Qualifications

• Licensed Physical Therapist required; new grads welcomed.
• Proficient in patient assessment and rehabilitation planning.
• Ability to design and implement discharge summaries and care plans.

• Screen and evaluate patients with physical functioning disorders and/or wounds.
• Deliver rehabilitative, skilled, and medically necessary treatments.
• Perform procedures as part of rehabilitation plans (manual therapy, ambulation, exercises, modalities, devices, wound care).

• Generate comprehensive discharge summaries meeting regulatory standards.

Pine Crest Health & Memory Care in Merrill, WI, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Key Responsibilities:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications:

Physical Therapist license is required. Open to Physical Therapists at all experience levels. New Grads are welcome!
