Emerald Hills Post Acute and Senior Living in Mount Horeb, WI is seeking a PRN Physical Therapist Assistant to join our in-house rehab program. You will screen and evaluate patients, develop care plans, and deliver rehabilitative services to support recovery.

Open to PTAs at all experience levels, including new grads, with a license required. This role requires standing/walking, lifting and repositioning, and offers a supportive team culture aligned with CAPLICO values.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wounds, and develop care plans.
• Deliver rehabilitative, skilled and medically necessary treatment interventions.

• Administer procedures as part of the rehabilitation plan, including exercises and wound care.
• Provide counseling to patients, families, and care teams regarding physical disorders.
• Generate discharge summaries compliant with regulatory and clinical standards.

Emerald Hills Post Acute and Senior Living in Mount Horeb, WI, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated PRN Physical Therapist Assistant for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications
