Angels Care Home Health in Mount Ayr, IA, is seeking a licensed Physical Therapist to join our full‑time team. You will plan, implement, and evaluate patient‑focused PT programs based on medical referrals and evaluations.

Collaborating with an interdisciplinary team, you ensure high‑quality care and adherence to DME knowledge, CPR standards, and state licensing requirements. The role offers Monday–Friday scheduling, a competitive compensation package, vehicle allowance, and performance bonuses.

Qualifications

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Plan, implement and evaluate patient care plans to restore or maintain patient well‑being.
• Plan, organize and conduct physical therapy treatment programs based on medical referrals and evaluations.
• Collaborate with interdisciplinary team.
