Job summary

Angels Care Home Health seeks a licensed Physical Therapist to assess and treat patients in home health settings. You will plan, implement, and evaluate care plans, coordinate with an interdisciplinary team, and adapt therapies to patient goals.

Full-time role with salary, vehicle allowance, and performance bonuses. The position requires a current Iowa PT license, CPR certification, and knowledge of durable medical equipment.

Qualifications

• Two+ years of PT experience in home health or clinical setting.
• CPR certification required.
• Knowledge of durable medical equipment and assistive devices.

Responsibilities

• Plan, implement, and evaluate patient care plans to restore or maintain well-being.
• Develop PT treatment programs based on referrals and evaluations.
• Collaborate with interdisciplinary team to optimize patient outcomes.

Skills
