Job summary

American Senior Communities is seeking a Physical Therapist for Monticello Healthcare and Hickory Creek Winamac to deliver skilled services to our senior residents. You will evaluate patients, develop initial and ongoing treatment plans, discharge plans, supervise PTAs, and contribute to case management.

The Physical Therapist implements skilled physical therapy services for patients in all ASC Therapies & Wellness settings, in accordance with the principles and practices of physical therapy and within the policies and procedures.
