Job summary

Premier Home Health Providers Inc. is seeking a Physical Therapist to provide per diem home health services in California. You will evaluate patients, develop individualized treatment plans, and deliver skilled PT per plan of care while educating patients and families.

The role requires an active CA PT license, CPR (AHA), and reliable transportation. Responsibilities include coordinating with physicians and the care team, supervising assistants when applicable, and handling clinical

• Active California Physical Therapy license is required.
• Minimum 2 years of PT experience; home health is a plus.
• Strong communication and empathy with patients and families.
• Current CPR certification (AHA).
• Valid driver's license and reliable insured vehicle.

Responsibilities

• Evaluate patients and develop individualized home PT plans.
• Deliver PT services following physician's plan of care and CA rules.
• Educate patients and caregivers on exercises and equipment use.
• Collaborate with physicians and care team; report changes.
• Supervise PT assistants and oversee HHA care when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify additional home health needs and coordinate referrals.
• Perform other duties as required.

Please note that this role is per diem with a rate of $160 - $180 per visit.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.
