Job summary

Premier Home Health Providers Inc. invites a skilled Physical Therapist to deliver in-home therapy services across California. You will evaluate patients, design plans, and work with physicians to support recovery in the home setting.

This per diem role offers flexible scheduling, mileage reimbursement, and potential to convert to full-time based on performance. A CA license, CPR, and 2 years of PT experience are required, with a focus on compassionate care for patients and families.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health preferred.
• Current CPR certification (AHA).
• Valid driver’s license and insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly when PT is sole skilled service.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

• Valid driver’s license and reliable, insured vehicle.
