Job summary

Premier Home Health Providers Inc. is seeking a Physical Therapist to join our compassionate team in a per diem capacity.

You will evaluate patients, provide skilled PT in home health settings, educate families, and coordinate with physicians to support recovery goals. Requirements include a Bachelor’s or Master’s in Physical Therapy, active California license, 2+ years PT experience (home health a plus), CPR, and a valid driver’s license with reliable vehicle.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy (ACLS not required).
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop treatment plans.
• Deliver PT services per physician plan of care and state requirements.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PT assistants and oversee Home Health Aide care when needed.
• Document progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $160 - $180 per visit.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.
