Job summary

P4 Physical Therapy is seeking a dedicated Physical Therapist to provide coverage in our outpatient clinic. You will assess patients, develop treatment plans, and deliver direct care following established guidelines.

Qualifications include CAPTE-accredited PT training, active license or readiness to start licensing, CPR, and strong communication skills. Outpatient orthopedic experience is a plus, and a compassionate, team-oriented mindset is essential.

Qualifications

• Graduate from a CAPTE accredited Physical Therapist program.
• Current state license or ability to begin licensing promptly.
• CPR certification.
• Outpatient orthopedic experience preferred but not required.
• Excellent communication skills to interact with patients, staff and other healthcare professionals.
• Strong multi-tasking abilities to manage the clinic effectively.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician referrals and patient records to determine PT treatment required.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions to patients and caregivers.
• Oversee patient care performed by assistants and rehabilitation technicians.
• Complete a written plan of care, set goals, and develop a comprehensive program.
• Implement PT treatment programs and provide essential treatments.
• Communicate with patients, caregivers, physicians, and insurers; ensure timely documentation.

P4 Physical Therapy is looking for a dedicated and skilledPhysical Therapist to provide coverage in our outpatient clinic.

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

• Graduate (or soon to be) from a CAPTE accredited Physical Therapist program
• Current statelicense or the ability to begin the licensing process promptly
• CPR certification
• Outpatient orthopedic experience, including exercise, manual-based skills, and sports rehabilitation, is preferred but not required.
• Excellent communication skills to effectively interact with patients, staff, and other healthcare professionals
• Strong multi-tasking abilities to manage the clinic effectively
