Job summary

P4 Physical Therapy is seeking a dedicated Physical Therapist to provide outpatient clinic coverage. You will assess patients, develop treatment plans, and deliver direct care following established protocols while coordinating with staff and physicians.

Responsibilities include documenting progress, communicating with caregivers and insurance providers, and ensuring timely notes and reports. A CAPTE-accredited program completion and active license or licensure eligibility are required.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapist program.
• Current state license or eligible for licensure promptly.
• CPR certification is required.

Responsibilities

• Assess patients and develop treatment plans.
• Review referrals and records to determine PT treatment.
• Provide direct patient care per protocols and guidelines.
• Document findings, progress, and instructions to patients.
• Coordinate care with assistants and rehab techs.
• Develop plan of care and goals based on evaluations.
• Implement therapy programs and essential treatments.

Job Description

P4 Physical Therapy is looking for a dedicated and skilled Physical Therapist to provide coverage in our outpatient clinic.

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports

Qualifications

• Graduate (or soon to be) from a CAPTE accredited Physical Therapist program
• Current state license or the ability to begin the licensing process promptly
• CPR certification
• Outpatient orthopedic experience, including exercise, manual-based skills, and sports rehabilitation, is preferred but not required.
• Excellent communication skills to effectively interact with patients, staff, and other healthcare professionals
• Strong multi-tasking abilities to manage the clinic effectively
• A self-starter with a mindset for achieving growth and success
• A passion for providing quality patient care with a positive and compassionate approach
