P4 Physical Therapy is seeking a dedicated Physical Therapist for full-time coverage in our Birmingham, AL outpatient clinic. You will assess patients, create treatment plans, and deliver direct care under established protocols, while coordinating with physicians and payors.

Qualifications

• Graduate from an accredited college with an APTA curriculum.
• Current state licensure in Alabama.

Responsibilities

• Assess patients to develop and initiate treatment plans based on evaluation results.
• Review physician referrals and patient records to determine required physical therapy treatment.
• Provide direct patient care by established protocols and guidelines.
• Document findings, progress, and instructions for patients and caregivers.
• Supervise assistants and rehabilitation technicians in patient care.
• Complete a written plan of care and set goals based on evaluations.
• Implement treatment programs and provide essential physical therapy interventions.
• Communicate with patients, caregivers, physicians, and insurers.
• Ensure timely documentation including daily notes and discharge summaries.

Job Description

P4 Physical Therapy is looking for a dedicated and skilled Physical Therapist to provide full-time coverage in our outpatient clinic in Birmingham, AL. If you are passionate about providing quality care and looking for a supportive work environment,

• Assess patients to develop and initiate treatment plans based on the evaluation results
• Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required
• Provide direct patient care by established protocols and patient care guidelines
• Document findings, progress, and instructions to patients and caregivers
• Responsible for patient care performed by assistants and rehabilitation technicians
• Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals
• Implement physical therapy treatment program and provide essential physical therapy treatments
• Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports
