Conduct comprehensive physical therapy evaluations and develop personalized treatment plans
Deliver evidence-based therapy sessions to improve strength, mobility, and functional independence
Collaborate with nursing, therapy, and care teams to ensure holistic care
Educate residents, families, and team members on therapy techniques and progress
Monitor outcomes, document progress, and adjust plans for optimal results
Promote a positive, respectful, and safe environment for all residents
Degree from an accredited Physical Therapy program
Valid Physical Therapist license in the state of Ohio
Strong communication skills and a team-focused mindset
A passion for improving the lives of older adults
Experience in skilled nursing, rehab, or long-term care is a plus
